How do I keep from stripping screw heads?

never used have problem, newer screws (philips) seems lose grooves easily. i've tried different screwdrivers , different sized screwdrivers , still happens. there technique avoid that?

, once it's stripped, suggestions on how remove such screw? dont want spend money on devices. i've used pliers, doesn't work.
